Application Forms and Membership Classifications

The WFO application form will open as an Adobe PDF form in a new window.  On most browsers, the form will open as a fillable pdf. If so, please fill in the form and print it before closing your browser. If your browser does not support fillable pdfs, you may have the option to open it in a different viewer such as Adobe Acrobat Reader DC, which will support fillable fields. 

                              

If not, you will need to print the form and fill it in manually.  Each application form requires a signature for “Verification of Eligibility” from the orthodontic association in which the applicant resides or is being trained, and for students, an additional “Certification of Enrollment” signature.  These signatures are mandatory to attest to the applicant’s status as an orthodontic specialist or as a student in an accredited orthodontic program as defined by a national association of orthodontists affiliated with the WFO. The applicant should obtain these signatures on the application form prior to sending the application to the WFO to allow the most rapid processing. 

To finalize your application you must print it, complete the financial information and signatures in writing. For your security please fax the completed form to the WFO secure fax line or you may scan the completed and signed application and send it via a secure encrypted e-mail service. Fellow (WFO member): This classification of membership is available to any orthodontic specialist who is a full member of a national or regional association of orthodontists that is affiliated with the WFO, provided he or she resides within the jurisdiction of such association. Individual orthodontic specialists who do not reside in a country with a WFO affiliate national orthodontic organization, may use this form to make a direct application to the WFO.

Academic Member: This classification of membership is available to a non-orthodontist engaged in full-time education or research relating to orthodontics at an accredited institution defined by a national or regional association of orthodontists affiliated with the WFO.

Student Member:   This classification of membership is available to a full-time orthodontic student or resident in an accredited orthodontic program as defined by a national association of orthodontists affiliated with the WFO.

Retired Member: This classification of membership is available to any former WFO Fellow who has retired from the active practice of orthodontics and/or retired from full time teaching or research.

Student to Fellow transition: This form is for student members who have graduated and now need to change to active membership.

Reinstatement form: This form has been developed to help facilitate colleagues requiring a WFO reinstatement.

Fellow change of Society transition: This form is for WFO fellow who has moved to a different country and needs to transition membership to a new Affiliate organization.

Honorary Fellowship : Any individual, orthodontist or not, who is deemed to have rendered outstanding service to the specialty of orthodontics may be nominated for WFO Honorary Fellowship
